INTRODUCTION


JONAC contains Diclofenac which belongs to a group of medicines called non-steroidal anti-inflammatory drugs (NSAIDs). It is used to relieve pain, reduce swelling and ease inflammation in conditions affecting the joints, muscles and tendons including rheumatoid arthritis, osteoarthritis, acute gout, ankylosing spondylitis backache, sprains and strains, soft tissue sports injuries, frozen shoulder, dislocations and fractures, tendonitis, tenosynovitis, bursitis. It is also used to treat pain and inflammation associated with dental, minor surgery and juvenile chronic arthritis, also be used alone, or in combination with other painkillers for the short term treatment of pain experienced after an operation (in children).


Diclofenac is an NSAID which exhibits anti-inflammatory analgesic and antipyretic activities. It involves inhibition of cyclooxygenase (COX-1 and COX-2) and it is a potent inhibitor of prostaglandin synthesis in vitro. Diclofenac is an inhibitor of prostaglandin synthesis; its mode of action may be due to a decrease of prostaglandins in peripheral tissues. It used to absorb rapidly into the bloodstream from the rich blood supply of the rectal area. When absorbed, it works by blocking the release of chemical messengers that cause pain and inflammation.


Always use this JONAC exactly as directed by your doctor. Before using this medicine, tell your doctor if you have liver, kidney or heart problems, are pregnant or breastfeeding. Your doctor will prescribe the right dose depends on your age and body weight. The common side effects are nausea, rectal irritation, headache and dizziness.



 








USES OF JONAC



  • Relieves pain, reduce swelling and inflammation in conditions such as rheumatoid arthritis, osteoarthritis, acute gout, ankylosing spondylitis, backache, sprains and strains, soft tissue sports injuries, frozen shoulder, dislocations and fractures, tendonitis, tenosynovitis and bursitis

HOW JONAC WORKS


JONAC works by blocking the action of enzyme cyclooxygenase, which is required for the biosynthesis of prostaglandins.



 








DIRECTIONS FOR USE


Always use JONAC exactly as your doctor has told you. It is for rectum use only and should not take them by mouth.



  • Unwrap and insert 1 suppository into the rectum as advised by your doctor

  • Lie down on your left side with right knee bent

  • Gently push the medicine into the rectum with your finger

  • Keep the suppository in the rectum to allow it to melt and the medicine to be absorbed

  • Remain lying down for a few minutes, and avoid having a bowel movement for an hour

  • Wash your hands after use



 








SIDE EFFECTS OF JONAC






SERIOUS



  • Sudden and crushing chest pain

  • Breathing difficulty when lying down, swelling of the feet or leg

  • Weakness, numbness in the face, arm or leg especially on one side of the body,  sudden loss or disturbance of vision, sudden difficulty in speaking or ability to understand speech, migraine-like headaches which happen for the first time, with or without disturbed vision

  • Wheezing or shortness of breath  

  • Swollen face, lips, hands or fingers

  • Yellowing of skin  

  • Persistent sore throat or high temperature

  • Change in the amount of urine produced or appearance

  • Chest pain (Kounis syndrome)



 








COMMON



  • Stomach pain

  • Stomach ulcers or bleeding

  • Gastritis (inflammation, irritation or swelling of the stomach lining)

  • Vomiting blood

  • Diarrhoea with blood  

  • Black, tarry faeces or stools

  • Fluid retention (swollen ankles)

  • Liver function disorders (hepatitis and jaundice)

  • Asthma (wheezing, breathlessness, coughing and tightness across the chest)



 





 








HOW TO MANAGE SIDE EFFECTS


Nausea:


Stick to simple normal meals and do not eat rich or spicy foods.


Stomach pain:


Place a warm water bottle on your abdomen. Drink plenty of clear fluids and water.


Diarrhoea:


Drink plenty of fluids to avoid dehydration. ORS should be used to prevent dehydration.


Vomiting:


Drink clear or ice-cold drinks. Eat light, bland foods and avoid fried, greasy, or sweet foods.


Dizziness:


Get up and move around to feel awake, take small naps to edge off the sleepiness.


Headache:


Rest and drink plenty of fluids. Apply a cold, wet cloth or ice pack to your forehead. Rest in a quiet, comfortable, dark room.



 








WARNING & PRECAUTIONS






PREGNANCY


JONAC is not recommended for use for last 3 months of pregnancy unless necessary. Consult your doctor for advice.



 








BREASTFEEDING


JONAC should be used with caution during breastfeeding unless necessary. Consult your doctor for advice.



 








DRIVING AND USING MACHINES


Do not drive or handle any machines while taking this medicine it may cause dizziness, tiredness, sleepiness or blurred vision.



 








KIDNEY


JONAC is not recommended in patients with severe kidney failure or any active kidney diseases. Consult your doctor for advice.



 








LIVER


JONAC is not recommended in patients with liver failure or any other active liver diseases. Consult your doctor for advice.



 








ALLERGY


Tell your doctor if you are allergic (hypersensitive) to Diclofenac.



 








OTHERS


Inform your doctor if you have or had,



  • Stomach or duodenal ulcer

  • Bleeding in the digestive tract

  • Blood circulation (peripheral arterial disease)

  • Ineffectual straining to empty the bowels

  • Diarrhoea or rectal bleeding

  • Bowel disorders such as ulcerative colitis or Crohn’s disease

  • Porphyria

  • Asthma

  • Chronic obstructive pulmonary disease, nasal polyps or hay fever

  • Diabetes

  • Angina

  • Increase in blood pressure

  • High cholesterol or triglycerides

  • Systemic lupus erythematosus (SLE)



 





 








INTERACTIONS


Inform your doctor if you are using or have recently taken any other medicines, including medicines obtained without a prescription or the following,



  • Aspirin or ibuprofen (used for inflammation or pain)

  • Warfarin, heparin (used to treat blood clot)

  • Medicines to treat diabetes

  • Diuretics

  • Lithium, serotonin re-uptake inhibitors (used to treat anxiety and depression)

  • Phenytoin (used to treat epilepsy)

  • Digoxin (used for heart problems)

  • Methotrexate, ciclosporin, pemetrexed and tacrolimus (used for inflammation and cancers)

  • Quinolones antibiotics (used to treat some infections)

  • Steroids (used to treat immune system)

  • Colestipol, cholestyramine (used to reduce cholesterol)

  • Voriconazole (used to treat fungal infections)

  • Mifepristone (used during termination of pregnancy)

  • Medicines used to treat heart conditions or high blood pressure (betablockers or ACE inhibitors)
